    The abuse of anabolic steroids by human athletes has far outshadowed their legitimate use in medicine. Anabolic steroids were originally derived from the male sex hormone testosterone but modern preparations are less masculinizing than the parent compound. Anabolic steroids are synthetic derivatives of testosterone having growth-promoting properties.

    Anabolic steroids have often been used in attempts to reverse the negative nitrogen balance associated with surgery, trauma, illness, and aging. Actual improved nitrogen balance is contingent on adequate protein/calorie intake and treatment of the underlying disease. What then are the indications for using anabolic steroids in pets.

    Anabolic steroids are most often prescribed by veterinarians for pets experiencing anemia, or a lack of red blood cells. Anabolic steroids stimulate a hormone, erythropoietin, which in turn stimulates the cells of the bone marrow to produce more red blood cells. Many months of therapy are often required to achieve the desired response. Patients that are anemic due to kidney failure (erythropoeitin is produced by the kidneys) may also benefit (as reported in people) but this has not been conclusively demonstrated in pets. Anabolic steroids may also be helpful when used in autoimmune diseases and together with chemotherapy in some cancers. These compounds may demonstrate a stimulatory or protective action on the immune response.

    The major side effect of anabolic steroids is liver toxicity. This phenomenon has been reported in dogs as well as people. Because of the potential masculinizing effects of these male hormone derivatives, effects seen in dogs can include increased sex drive, interference with normal female reproductive cycling, masculinization of the fetus during pregnancy, increased prostate size and stimulation of male hormone-dependent tumors of perineal glands, glands of the anal sacs, or prostate. Anabolic steroids should also not be used in patients with congestive heart failure because they cause retention of sodium and water.

    Until controlled studies in pets are available, anabolic steroid therapy should be implemented with caution and should not be substituted for proper treatment of the underlying disease.
